There are many tools (besides exhausting empathy injections) you can use to keep the doctor-patient relationship far from the Zone of Peril. How common are the personality disorders? According to a recent meta-analysis (Volkert, 2018), over 12% of your patients likely have one. The rates are much higher among those in your practice with “illness anxiety disorder”: 75% of hypochondriacs have one personality disorder, and nearly 50% have three or more. Unfortunately, and as you know all too well, personality disorders aren’t found only among our patients. The personalities of our colleagues, family members, and friends can also contribute to some rather profound suffering. This practical workshop begins with a review of the science of personality assessment. We examine the most popular inventories, doing a fair bit of debunking along the way. Fortunately, some inventories are indisputably evidence-based. You'll have an opportunity to analyze yourself (and your loved ones, if you're so inclined) using one of the very best. The core of the workshop is the systematic review of DSM-5’s ten personality disorders: the insensitively-named "MAD" (paranoid, schizoid, schizotypal), "BAD" (antisocial, borderline, histrionic, narcissistic), and "SAD" (avoidant, dependent, and obsessive-compulsive) "clusters". As we review each of the ten personality disorders, our emphases are on 1) rapid diagnosis (when possible); 2) modular treatments (when desired), and 3) clinician coping (always). The complex issue of the diagnosis of children and adolescents will be debated. Borderline personality disorder necessitates extra time. Today nearly 20% of female university students have significant BPD symptoms, and cutting is rising among tweens (ages 8 to 12). In this expanded section we focus on managing non-suicidal self-injuries, with practical tips harvested from CBT and its relevant derivatives (i.e., DBT and ACT). Although nobody woke would advocate labeling, there is clearly much value in knowing what kind of person one is dealing with. Many case challenges (e.g., in preventive medicine & chronic disease management)—and a high percentage of interpersonal disputes—gain clarity through the lens of personality. Personalities and their disorders are by definition enduring and predictable. When you improve your skills in reading others, you give yourself a little "superpower". That superpower will make both your clinical practice and your life in general a fair bit easier.

The 8th International Congress of Clinical and Health Psychology in Children and Adolescents is a scientific meeting where the latest challenges in child and adolescent psychological assessment, psychopathology and treatment are addressed. The congress is organized by the AITANA Research Group (Analysis, Intervention and Applied Therapy with Children and Adolescents) of Miguel Hernández University (Spain). Around 600 academics and professionals from over 50 countries have participated in previous editions. Aaron Beck (1921–2021) published his first book on depression in 1967, the year the Maple Leafs last won the Stanley Cup—and when many of today’s physicians were just hopeful helices of DNA. Just like you, CBT has evolved immeasurably over the past 55 years. We present “CBT for Depression, Version 2022”, an approach that is very different—and much more effective—than Beck's 1967 package. In the shell of a nut, CBT for MDD has become more behavioral and less cognitive—and the research to support the pendulum swinging is large and incontrovertible. Topics include reasons for optimism; empathy addiction; goalification, scaling, and I/O charts; behavioral antidepressants; pathogenic belief identification; cognitive illusions; the physics of CBT; therapeutic persuasion; thought records; the CUE question (to harness emotional intelligence); learned helplessness (and its opposite, learned hopefulness); reductionistic stagnation; future-focusing (intertemporal self-state negotiations); and the lay construct of willpower. The Depression module provides physicians with a comprehensive kit for more effectively managing one of the most common human afflictions.

McGill University Faculty of Medicine is again sponsoring two medical CBT workshops at the Annual Refresher Course for Family Physicians: Personality Disorders: Manage them before they manage you and GriefWork: Growing from life's inevitable losses. Both workshops assume that appointments are ten minutes or less in duration. As such, the focus is exclusively on those tools that are highly practical and efficient. Although ideally taken together for 18.0 Mainpro+ credits, the workshops can also be taken individually for 9.0 Mainpro+ credits each. Losses happen. That's always been true, but it's more salient in these dreadful Days of COVID. Historically, it was the wisdom traditions—religion and philosophy—that provided us with comfort. Today psychology helps too: the utterly universal experience of loss has spawned much excellent scientific research. This workshop assumes your appointments are brief, averaging only 5–7 minutes. With that in mind, we teach the CBT tools that are highest in impact and practicality. The goal is to efficiently help patients cope—at least a little better—with the pain of some of life’s inevitable losses.

Today’s cognitive behavior therapy (CBT) is a treasure-trove of scientifically‐tested tools to help physicians increase their resiliency and reduce their risk of burnout. Physician wellness is important for countless reasons, including that it significantly determines the quality of care delivered (CMPA, 2018). Simply put, the burned-out doctor is probably a crappy doctor. Sadly, the burned-out are also at risk for Premature Clinician Aging Disorder (PCAD). PCAD isn't just the result of a heavy caseload and long hours. PCAD is also predicted by one's psychology. PTSD: From post-traumatic stress to post-traumatic growth (2023.06.07; via Zoom)

PTSD has a lifetime prevalence rate of nearly 10% in women (5% in men), which rivals that of depression. Fortunately, the DSM-5 "holy book" made it easier to diagnose PTSD, and recent research has provided us with much more effective treatments.

This popular workshop begins with a review of debriefing (i.e., interventions designed to prevent PTSD after a trauma), including insights from the lead presenter’s work with the Canadian government's National Roundtable in Ottawa. We then review a shortcut to the diagnosis of PTSD, proceeding from there to the details of (and controversies pertaining to) the fuller diagnostic criteria. The core of the workshop—training in treatment—emphasizes those tools realistic for use in primary care. As always, the assumption is a ten-minute appointment (with three patients in the waiting room). Special topics include the curious case of EMDR (Eye Movement Desensitization and Reprocessing), “emotional baggage” (i.e., old traumata), the eye-opening forgiveness research, so-called "hindsight bias", the Oprah-esque "imaginal rescripting", intergenerational trauma, the Faustian bargain of children who survived ACEs (adverse childhood experiences), complex PTSD (C-PTSD), trauma-informed care, and what data we have on medical marijuana. The workshop ends with a very uplifting topic: the emerging and exceedingly hopeful literature on post-traumatic growth.
